JAX NEMT is a licensed and insured non-emergency medical transportation provider based in Jacksonville, Florida. This policy explains what information we collect when you contact us or ride with us, how we use it, and the choices you have. This page is maintained by JAX NEMT and describes our own practices.

Information we collect

When you call, text, email or submit a request, we collect the information you give us: your name, phone number, email address, pickup and drop-off addresses, appointment times, mobility and equipment needs, and any escort or caregiver details you share. We collect payment information only as needed to process payment for your trip.

When you visit this website, standard technical information such as your browser type, device type and pages viewed may be recorded by our hosting provider for security and performance purposes.

How we use your information

We use your information to schedule, dispatch and complete your trips, to contact you about pickups and changes, to quote and bill for service, to keep records required for licensing and insurance, and to answer your questions. We do not sell your personal information.

Sharing your information

We share only what is necessary with our drivers and dispatch staff. When a trip is coordinated through an independent transportation network or a partner provider, we share the trip details needed to complete that ride. Those companies operate independently and their own privacy policies apply to information you submit directly to them. We may also disclose information when required by law or to protect the safety of riders and staff.

Cookies and analytics

This site uses only the cookies and storage needed for the pages to work correctly. If we add analytics in the future to understand which pages riders find helpful, we will use it in aggregate and will not use it to identify individual riders.

How we protect and retain information

We limit access to rider information to the staff who need it to do their job, and we keep trip and billing records for as long as required for accounting, insurance and licensing purposes. No method of transmission or storage is completely secure, so please do not send sensitive medical details by email or text beyond what we need to plan your ride safely.

Your choices

Children's information

We transport pediatric patients, and any information about a minor is provided by a parent, guardian or caregiver and used only to arrange and complete their transportation.
